Chelsea must win their final group game against FC Nordsjaelland at Stamford Bridge and hope that Shakhtar can beat Juventus in Donetsk to avoid becoming the first holders to be eliminated in group stage.
Should Juventus draw that game or even win it, Chelsea will be out of the competition.
